Welcome to the Health Information Management (HIM) Department!

This department was formerly known as Medical Records. Because we are concerned about the security of your health information, it is managed in the most confidential and secure manner as possible.

 

The HIM Department is composed of Medical Transcriptionists, Coding Specialists, an Office Support person, and a supervisor. Each team member is dedicated to provide caring and quality customer service to North Valley Hospital patients and providers who serve our communities.

 

Medical Records Requests:

We strive to make access to your medical records as easy as possible for the continuation of your care. To request a copy of your medical records you will need to complete an Authorization for Release of Healthcare Information. You can find that form here. Once that authorization has been received, the request will be completed and mailed to the provider as stated on the authorization within 14 days.

 

Birth Certificate Program:

We offer complimentary Hospital Birth Certificates to babies that are born at North Valley Hospital. Although this is not a legal document, it provides a family record of the child’s birth, which is complete with a copy of the photo that is taken for the “Baby’s First Web Page”.

 

Legal Certificates of Birth for all children that were born in Washington State after 1939 are available through the Okanogan County Health District.
