 Medical Records Requests

We strive to make access to your medical records as easy as possible for the continuation of your care. To request your medical records you will need to complete an Release of Medical Information Form. Once that authorization has been received, the request will be completed and mailed to the provider as stated on the authorization within 14 days.

We are dedicated to keeping your medical information confidential, which is why we need a completed and signed Release of Medical Information Form in order to release your records. 

To request copies of medical records, please print the release form, complete, sign, and return to:

Email Requests: If you would like your medical records sent electronically send an email to HIM@nvhospital.org. Include your full name (first, middle, last) and date of birth. Include what records you are looking for with the date(s) of service, if possible. Emails will be sent encrypted. Fees may still apply.

Phone Requests: Medical Record requests can be made via phone call to the HIM department (509) 486-3181. However, if the request is for electronic copies please refer to Email Requests.
